How To Request A Signature/Avatar

Signature, Avatar, or Both?: Do you want an avatar to go with your signature?
Theme: Do you want a chicken theme? Monkeys? Call of Duty? Halo? Seriously can be anything.
Preferred Colour(s): What colours do you want involved? Maybe just one or many.
Required Text: Usually people ask for their names in the images, but some may want sayings or something else.
Dimensions: Size of images you want. Standard signature size is 400px × 175px and standard avatar size is 100px x 100px.
Preferred Style: Overall kind of image. Artsy? Grudge? Simple? Etc.
Preferred Artist: Do you want someone or do you have an idea of a few people you would like to request from?
It Must Include The Following Images: Outside images that you'd like to be included within your request banner.
Other Comments/Notes: Whatever doesn't fit into those other categories.

So... If I wanted to make a request, I would just fill it out like this...
Signature, Avatar, or Both?: Just a signature, please.
Theme: Spacey. Nothing particular.
Preferred Colour(s): Blue and green! Can you make it bright?
Required Text: Just my name, please. 'crait'
Dimensions: 400px × 144px
Preferred Style: I like a lot of other signatures that have aura beams. Can you include some of those?
Preferred Artist: No one particular.
It Must Include The Following Images:
Other Comments/Notes: Nothing else.
